Motor carrier forms and publications

All forms are in Adobe PDF format unless otherwise noted. Use these PDF forms in one of two ways. You may either:

• print the form and then write in your information, or
• you may type your information into the fields provided on the form and then print, sign and mail it to the address on the form. Each form contains fields for you to type in your information. After selecting the form, click in an appropriate field to make an entry. You may use your tab key to advance to the next available field.

• MV2755 - Wisconsin Fuel Tax Quarterly Report for IFTA: These forms are in MS Word format.
  • 4th quarter 2012 - Diesel fuel only - due by January 31, 2013
  • 4th quarter 2012 - Non-diesel fuel only - due by January 31, 2013
  • 3rd quarter 2012 - Diesel fuel only - due by October 31, 2012
  • 3rd quarter 2012 - Non-diesel fuel only - due by October 31, 2012
  • 2nd quarter 2012 - due by July 31, 2012
  • 1st quarter 2012 - due by April 30, 2012
• MV2766 - 2013 Wisconsin International Fuel Tax Agreement License Renewal
• MV2940 - WisCRS System Access Request - This is the access agreement and application for access to our Internet application that allow for the electronic submission of IRP applications and IFTA fuel tax reporting.
