Replacement titles

Application

If your Wisconsin certificate of title has been lost, destroyed, or stolen, you may obtain a replacement by filing a Title & License Plate Application form MV2119.

Please note that, beginning July 30, 2012, state law requires the department to deliver all titles to the lien holder (lender). If you have a lien on your vehicle, check Lien look up and contact the lien holder to obtain the title, if the date processed is on or after July 30, 2012.

What is needed

Only the owner, or first listed lien holder after July 30, 2012, shown in Division of Motor Vehicles (DMV) records may apply for a replacement title. The application must include the vehicle identification number and the signature of the owner, or lien holder if applicable. The vehicle identification number can be found on your registration card, or on the vehicle itself. Mail the application to the address on the application form. The fee for a replacement title is $20.

OR

You may bring the application to any DMV service center offering title and registration services for immediate processing for an additional $5 counter service fee. You will need to show proof of identity in the form of a driver license or identification card when applying for a replacement title. You will receive the title the same day. DMV customer service centers accept cash, checks or money orders.

Permission to pick up title is required

DMV requires anyone picking up a replacement title on behalf of the owner or lien holder to present a completed "Permission to Pick Up Title" form MV2932 (Spanish version), and to show proof of identity in the form of a valid driver license or identification card. This form is in addition to the MV2119 (Replacement Title Application) that should be completed by the owner, or first listed lien holder after July 30, 2012, in DMV's records. If the requestor does not have written permission or proper identification, the Wisconsin Department of Transportation (WisDOT) will mail the title to the vehicle owner or lien holder.
