Adopt-A-Highway - Forms

Fill out the desired form below and email it, or send it via mail, to the appropriate regional coordinator.

The following document(s) are in MS Word format.

• Application/Permit (DT1206) - Fill out this form to apply for adopting a highway to help clean up.
• Clean-up report (DT2214) - Fill out this report once the clean up activity has been completed.
• Freeway/Interstate/Interchange Work Plan (DT1207) - Fill this form out to assure a safe clean-up of the freeway/interstate/interchange that you have adopted. (285 KB)
• Notice of abandonment (DT1205) - Fill this form out when your group decides to abandon the clean up activities.
  (Note - Please submit form 30 days prior to abandoning clean up activities.)
