County Elderly and Disabled Transportation Assistance 

The County Elderly and Disabled Transportation Assistance program provides counties with financial assistance to provide transportation services to elderly persons and persons with disabilities.

Please note 2012 annual financial reports are due March 15, 2013.

Please submit an updated trust fund plan whenever your plans for using your trust funds within the current calendar year change.

The county plan contact list is derived from the 2008 coordination plans. Many counties did not have a contact listed, and in most of these instances the individual from the 85.21 applications from County Departments of Health and Social Services, ADRC’s or Aging units is listed as the contact.
